Compliance Packaging

What is compliance packaging?

Compliance packaging, also referred to as adherence packaging, is a way to organize and arrange a patient’s medications based on prescribed directions and frequencies. Blister packing is one of the most popular compliance packaging formats which arranges doses into blisters based on the prescribed administration schedule. In addition, blister cards include important information such as date, time, drug names, imprint, pill color, dosage etc. to ensure accurate and timely administration.

Do you need compliance packaging?
  • Do you take multiple medications?
  • Are you overwhelmed by how and when to take your doses?
  • Is it hard to remember if you took a dose when scheduled?
  • Are you tired of running to the pharmacy to pick up refills?
  • Does your doctor change your medications often?

If you answered yes to any of these questions, the Tulalip Clinical Pharmacy’s Medbox service could help you.

What can Tulalip Clinical Pharmacy Blister Cards do for you?
  • All prescriptions arranged by dose and time of day
  • See at a glance when and what doses were taken
  • Eliminate chance of missing or doubling doses
  • One single trip to the pharmacy per month
  • Accurately and professionally prepared using state-of-the-art Synmed Blister Packing robot

Overdose Prevention

On June 10, 2010, Washington state enacted the “911 Good Samaritan” law (ESB 5516), which provides immunity from drug possession charges in drug overdose situations and expands access to naloxone, a prescription drug that reverses overdoses caused by opioids. The law authorizes any person to obtain and use naloxone, given the fulfillment of certain requirements, and authorizes any person to administer naloxone to a qualifying third party.

Tulalip Clinical Pharmacy is driven to raise awareness around naloxone and to increase access to this potentially life-saving medication. In 2015, Tulalip Clinical Pharmacy established a drug protocol to distribute naloxone, an emergency medication that will reverse opioid-induced overdoses. Our pharmacy staff, in conjunction with the Tulalip Chemical Dependency/Behavioral Health teams, organized outreach events to educate and train on the proper usage of naloxone kit in the event of an opioid overdose. In addition to distributing naloxone directly to patients and their family members, Tulalip Clinical pharmacy also supplied naloxone kits and provided training to other health care providers, fish and wild life staff, and local first responder teams.

We realize the urgency of the opioid epidemic and are committed to work with all involved parties as a united front against the opioid epidemic.

Below is a link to an interview of Gina Skinner of Family Services, Jane Jacobson from the Tulalip Pharmacy, and community activist Rico Jones Fernandez discussing the epidemic of opiate addiction, the Good Samaritan Law, and the use of the overdose antidote Narcan (naloxone).

Medication Counseling

Personal Medication Counseling at Tulalip Clinical Pharmacy

We Are a Teaching Pharmacy

Knowledge is power. Medications prescribed by your doctor are often confusing, especially when you are taking more than one prescription at a time. We believe it is important that patients know about their medications to really benefit from the therapy. Many people take two or more drugs, and need to know if there are interactions between them. Some medications have serious side effects that are important to know how to manage or avoid.

Help Us Teach You

At the Tulalip Clinical Pharmacy, we have two private counseling rooms. There we can sit down and privately discuss your medications, and answer questions you may have about them. Not only do we counsel on medications, we also demonstrate:

  • Inhalers and spacers
  • Insulin injection devices
  • Blood glucose monitoring kits
  • Nebulizers
  • Blood pressure measuring machines
  • And more

Pharmacists have consistently been rated among the most trusted and accessible health care professionals. This is a reputation we respect. We strive to be worthy of it by reliably counseling with our patients about how to effectively use their medications and medical devices.

Medication Therapy Management (MTM)

What Does MTM Cover?

Medication therapy management review, or MTM for short, covers a spectrum of services that are used by a pharmacist to optimize and individualize the best treatment outcomes for each patient. It is a process of collecting patient information, communicating with health care providers, assessing appropriateness of current or missing therapy, identifying medication related problems and developing a plan to solve them.

What Doesn't MTM Cover?

MTM reviews focus on getting the right drug to the right patient and the right time. MTM does not include the dispensing of medication.

MTM does not take the place of your primary care provider. Primary care providers will still supply diagnosis, education, medical procedures and make the final decisions that are necessary for each patient’s illness or disease. The MTM review focuses on the appropriateness of the medications that are being prescribed and allows the pharmacist to make recommendations for the solution of medication problems that arise.

Substance Abuse

Medication Monitoring

Drug abuse is a growing problem in Washington State where it is estimated that almost 12% of teens use medicines for non-medical reasons.1 The Tulalip Clinical Pharmacy works with prescribers to help ensure that their patients receive the right medicine at the right time. This one-on-one counseling at the pharmacy entails observed administration of disulfiram (Antabuse®) or buprenorphine/naloxone (Suboxone®). Pharmacists at the Tulalip Clinical Pharmacy find it rewarding to play a small part in assisting patients go from chemical dependence to individual independence as they make positive changes in their lives.

Antabuse® Direct Observation Program

Patients are prescribed disulfiram (Antabuse®) to maintain alcohol abstinence. In order to ensure patient compliance with the treatment regimen, it is often prescribed to be taken under the direct observation of a health care provider or other care taker. At the Tulalip Clinical Pharmacy there are private counseling rooms where a program patient meets with a pharmacist who administers the medication and gives counsel about the treatment regimen. The encounter is documented in the clinic-based electronic health record for patient and provider convenience.

Suboxone® Counseling Program

Pharmacists at the Tulalip Clinical Pharmacy play an important role in these programs that are set up by the health care providers. When patients receive Suboxone® treatment to assist their efforts in quitting a dependency on a substance they receive private one-on-one counseling. FAQs

Pharmacies cannot accept returned medications. Please see the next question for what you can do with your unused medication.

Unfortunately, Washington is lacking a statewide program that addresses this issue. In the meantime, the following links will provide you with current information about what you can do.

The Pharmacy has containers for you to use at home to collect your used needles, syringes and other sharps. There are also recycling and disposal stations and public health sites that accept these materials www.seattle.gov.

Yes, in most cases a pharmacist may transfer the remaining refills to another pharmacy. However, once your prescription has been transferred from the original pharmacy your prescription is now void and may no longer be filled there, unless you have it transferred back to the original pharmacy. Controlled medications may only be transferred once.

A pharmacist may give you a copy of your prescription only for informational purposes. A pharmacist may NOT dispense a drug from a copy of a prescription. A pharmacist may use the information on your copied prescription to contact your doctor for a new prescription.

No, a prescription may only be faxed to the Pharmacy by the healthcare provider.

The Pharmacy is only allowed to dispense the quantity that your doctor indicated on your prescription at a time. Also, insurance companies have their own limitations on refill quantity and frequency. For example, many insurance companies limit refills to a one-month supply. Prescriptions may only be refilled after the appropriate amount of time has lapsed in between refills.

Only a physician (MD), dentist (DDS), podiatrists (DPM), Advanced Registered Nurse Practitioners (ARNPs) and Osteopathic Physicians (DO) may issue a prescription. These physicians can issue prescriptions without any type of restrictions.

Physician’s Assistants (PA) and Certified Physician's Assistants (PA-C) may issue prescriptions only as approved by the Medical Commission. Pharmacists (RPh, PharmD) may write prescriptions only under an approved protocol agreed upon by the physician. This agreement must be filed with the Board of Pharmacy. Veterinarians (DV) can issue prescriptions to animals only, however these prescriptions can be filled at local pharmacies.

Certified Osteopathic Physicians Assistants (OA) and non-certified Osteopathic Physicians Assistants (IO), Certified Registered Nurse Anesthetist (CRNA), Neuropathic Doctor (ND) and Optometrist (OD) all are able to write prescriptions with certain restrictions.

In order to verify the correct identity of the person submitting and picking up prescriptions and for your safety, we collect and keep on file your social security number, date of birth and a photo copy of your ID. All of this information is stored securely and is only available to Pharmacy employees with a need to know the information.

A pharmacist may not refill a prescription unless authorized by your doctor. Your doctor may authorize no refills, one refill or several refills on the original prescription. After all of the refills on the original prescription have been used, the pharmacy must obtain a new authorization from your doctor before refilling your prescription.

Even though your doctor may want you to continue to take the prescription for a long period of time, he/she will monitor your drug therapy requiring the pharmacy to contact your doctor each time your prescription needs to be renewed

To avoid delays, give the Pharmacy a call a few days before your medication runs out. This allows the Pharmacy time to get in contact with your doctor and for the doctor’s office to respond to the Pharmacy.

Insurance companies limit the drugs that they will provide reimbursement for, or “cover”. Generally speaking, when the medications are essentially the same, the insurance companies prefer patients to try generic versions before brands. In some cases, your doctor will have to write a “prior authorization” letter to the insurance company in order to cover more expensive medications for you.

To find out if there is a generic option for a drug you are taking, go to DRUGS@FDA catalog of Food and Drug Administration (FDA) approved medications.

Go to www.dshs.wa.gov, or if you are a client calling for Medicaid information, you may contact DSHS by phone at 1-800-562-3022.

Tulalip Prescription Formulary and Coverage Guidelines

The cost of prescription drugs has increased dramatically over the past decade and become a tremendous responsibility for the Tulalip Tribe, especially when there has been no restriction or limit on prescription coverage. This prescription coverage has been funded through the Tulalip Prescription Benefit Program, which is a "payer of last resort" program. In an effort to ensure long-term sustainability of the prescription benefit and to provide the best and safest health care for our members, the Board approved a resolution in March of 2015 to create the Tulalip Prescription Formulary and Coverage Guidelines.

Resolution No. 2015-063 approved on 3/7/2015 by BOD

The Tulalip Prescription Benefit is required to be the payer of last resort. This means it will cover any remaining prescription cost when all available primary coverage sources are billed first. In other words, members must make reasonable effort to enroll and maintain primary coverage to receive Tulalip Prescription Benefit.

A formulary is a preferred list of medications intended to help guide prescribing decisions, developed using evidence based guidelines and best practice recommendations from multiple groups.

Formulary drugs were carefully selected by clinic providers and pharmacists. These drugs are chosen based on efficacy, safety, and cost-effectiveness.

Members are required to maintain active primary insurance or to have a pending primary insurance application before their prescriptions will be covered by the Tulalip Prescription Plan. In addition, your prescriptions must meet any restriction or limitation imposed by your primary insurance. Prescriptions may be dispensed up to the quantities allowed by member’s primary insurance. There will also be a limit to the frequency and total cost of covered replacement for lost or stolen prescriptions.

You or your pharmacy will contact your provider to request a switch to an equivalent product that is covered by the formulary. If your provider decides none of the formulary equivalent products are suitable for you, he/she can submit a prior authorization request to the Tulalip Pharmacy and Therapeutics Committee.

If your provider decides it’s in your best interest to continue your current medications, he/she can submit a prior authorization request to the Tulalip Pharmacy & Therapeutics Committee.

A prior authorization request is how your doctor may ask for a non-formulary medication to be covered by the Tulalip Benefit. This request will be reviewed by a group of pharmacists and providers who will decide if the request is medically justified. If the request is not approved, your doctor may choose a different medication that is covered by the formulary. The prior authorization process usually takes 2–7 days, depending how quickly your provider submit the required documentations.

There are first-step medications that are high quality and good values. These medications are preferred choices to try first. If a first-step medication doesn't work well for you or doesn't agree with you, there are other medications that can be tried next. Often, the first-step medications are generics that have been proven safe and effective for many patients over many years.

Beginning May 2015, detailed information about the new formulary and prescription coverage guidelines will be available at the Tulalip Health Clinic, Tulalip Family Services, Tulalip Clinical Pharmacy, and on this Tulalip Clinical Pharmacy website. In addition, the Tulalip Clinical Pharmacy staff will prepare personalized notification letters to affected members. These personalized notification letters will help members understand how they might be affected and what we can do to resolve coverage issues.

Yes. Tulalip Prescription Coverage covers only prescriptions written by outside providers if the prescription is in the Tulalip Prescription Formulary and if they are first billed through your primary insurance. Some drugs might be restricted to outside providers who meet certain criteria. For example, Buprenorphine (Subutex) and Buprenorphine/Naloxone (Suboxone) prescriptions will only be covered if they are prescribed by Tulalip Health System providers or by outside SAMHSA-certified providers who provide DBHR-approved consultations.

*SAMSHA= Substance Abuse and Mental Health Services Administration, DBHR=Division of Behavioral Health and Recovery.

Please feel free to contact the Tulalip Clinical Pharmacy staff if you have any question regarding the new Formulary and Coverage Guidelines. You can reach us by phone 360-716-2660 or by email (pharmacy@tulaliptribes-nsn.gov).

Policies

Permission to pick up prescriptions 4.28.17

Protecting patient’s privacy is one of the top priorities of Tulalip Clinical Pharmacy. Tulalip Clinical Pharmacy will only release prescriptions to patients or their authorized representatives.

ID requirement

Patients or their authorized representatives must present valid picture ID at the register. For dependent children (18 years old and younger), parent or legal guardian may pick up medications with their own ID Card. Tulalip Clinical Pharmacy reserves the right to exercise professional judgment in determining when a parent or guardian may not be a minor’s personal representative, such as when a minor has the legal right to consent to care, when a minor obtains care pursuant to court order, or by agreement of a parent or guardian. In such cases, Tulalip Clinical Pharmacy may be prohibited from disclosing a minor’s health information. Tulalip members are also required to present their Tulalip ID when prescriptions are processed through the Tulalip Prescription Plan. Tulalip Tribal Members will not be required to show picture ID if a copy of their ID is already on file with the Pharmacy to verify identity.

To authorize pick-up by another person:
  • Patient or legal guardian must fill out and sign the Prescription Pick up Authorization Form. Pick-up Authorization will remain valid throughout the calendar year and must be renewed after December 31st of each year. Patient can also revoke it at any time.
  • Patient or legal guardian can also authorize a one-time pick-up over the phone. Pharmacy staff will make a good faith effort to confirm identity of the caller and document the authorization in patient’s profile.
  • Pharmacy Director may use discretion to deny or revoke a pick up authorization if he/she feels patient’s care or privacy will be compromised.
